I have never perma deathed a character. On a world now dead my character there and her beloved rode to parts unknown.
On one of the current words I play, they decided to roll back the world in time. Several offers were made to have people stay. My two characters I had became npc's. One is a merchant and the other a master sergeant in the legion who gives out quests.
Would I permadeath, I really do not know. I try and change my character's with life experiences. More than likely i would retire them in case I wanted to return.

Notably, a cleric of Bane called Hiepherus. He was captured by heroes, tortured, killed (?) and thrown off a cliff into the ocean.
Kind of reminds me of:

Raven my first character on Thain, he died eaten by the razorfangs.
he died when I decided to stop videogaming for awhile (3-4 years) he stayed dead ever since, I like to think he arrived in the Thain's Halls of Mandos with the other Eldar and Aribeth
I am sad to have him gone from Thain, luckily he still lives in my stories and as my char on tabletop D&D.
