This is a known issue, currently the temp fix I have found is to back out of the detection zone and then reenter or approach from a different angle.

Blade Thirst when used on a ranged weapon does add the extra damage, but only for a round or two

Really? I've never had a problem with it working as expected. Are you using it on the weapon (ie: clicking the weapon in your inventory) or clicking on your character to cast it? I always have to do the latter. And you will see the extra damage on your ammunition, not on the weapon itself.
